ABSTRACT

Objective:To analyse the clinical characteristics and antiviral therapy in immunosuppressed hospitalized patients with influenza. Methods:The clinical data of 273 patients with positive influenza A or B virus nucleic acid admitted in Peking University People’s Hospital from November 2015 to March 2022 were retrospectively analyzed. Among them, 123 were immunosuppressed and 150 were non-immunosuppressed. The clinical characteristics and antiviral therapy in immunosuppressed patients with influenza were analyzed. SPSS 22.0 software was used to analyze the data. Results:Chemotherapy for malignancies was the most common cause of immunosuppression (61.8%, 76/123), followed by haemopoietic stem cell transplantation (24.4%, 30/123). The common symptoms were fever (93.5%, 115/123) and cough (41.5%, 51/123). The proportions of co-infections (22.8%, 28/123) and complications (43.9%, 54/123) in immunosuppressed hospitalized patients were higher than those in non-immunosuppressed patients ( χ2=9.365 and 7.496, both P<0.01). Compared with single drug therapy, combination of antiviral drugs did not shorten the fever time, negative conversion time of virus nucleic acid and the length of hospital stay, and reduce the death ( U/ χ2=312.5, 356.0, 749.5 and 0.185, all P>0.05). Compared to patients without corticosteroids use, the use of corticosteroids did not increase mortality in immunosuppressed patients ( χ2=2.508, P=0.113). Conclusions:Classical symptoms may be absent in immunosuppressed patients with influenza, and early detection of influenza virus is still an important means of early diagnosis. Co-infections and complications are more common in immunosuppressed influenza patients. Immunosuppressed influenza patients did not benefit from the combination of antiviral therapy.

ABSTRACT

Objective: To examine the feasibility, safety, and efficacy of mobilization of the vertebral artery for C2 pedicle screws in cases with high-riding vertebral artery (HRVA). Methods: The clinical data of 12 patients with basilar invagination and atlantoaxial dislocation underwent atlantoaxial reduction and fixation in the Department of Neurosurgery, the First Affiliated Hospital of University of Science and Technology of China between January 2020 and November 2021 were retrospectively analyzed. All patients had high-riding vertebral artery on at least one side that prohibited the insertion of C2 pedicle screws. There were 2 males and 10 females aged (48.0±12.8) years (range: 17 to 67 years). After correction of vertical dislocation during the operation, the C2 pedicle screw insertion and occipitocervical fixation and fusion were performed using the vertebral artery mobilization technique. Neurological function was assessed using the Japanese Orthopedic Association (JOA) scale. The preoperative and postoperative JOA score and the main radiological measurements, including the anterior atlantodental interval (ADI), the distance of the odontoid tip above the Chamberlain line, the clivus-canal angle, were collected and compared by paired t-test. Results: Mobilization of the high-riding vertebral artery was successfully completed, and C2 pedicle screws were then fulfilled after the vertebral artery was protected. There was no injury to the vertebral artery during the operation. Meanwhile, no severe surgical complications such as cerebral infarction or aggravated neurological dysfunction occurred during the perioperative period. Satisfactory C2 pedicle screw placement and reduction were achieved in all 12 patients. All patients achieved bone fusion 6 months after surgery. No looseness and shift in internal fixation or reduction loss was observed during the follow-up period. Compared to the preoperative, the postoperative ADI decreased from (6.1±1.9) mm to (2.0±1.2) mm (t=6.73, P<0.01), the distance of the odontoid tip above Chamberlain line decreased from (10.4±2.5) mm to (5.5±2.3) mm (t=7.12, P<0.01), the clivus-canal angle increased from (123.4±11.1) ° to (134.7±9.6) ° (t=2.50, P=0.032), the JOA score increased from 13.3±2.1 to 15.6±1.2 (t=6.99, P<0.01). Conclusion: The C2 pedicle screw insertion assisted by mobilization of the vertebral artery is safe and considerably effective, providing a choice for internal fixation in cases with high-riding vertebral arteries.

ABSTRACT

Objective:To understand the current status of smoking attempt among adolescents in Jiading District of Shanghai and to explore the influencing factors so as to provide suggestions for further intervention and control of tobacco smoking among adolescents. Methods:A total of 3 745 students from grade one to grade three from seven junior high school in Jiading District, Shanghai, were selected to participate in the survey by stratified cluster random sampling method. Anonymous internet questionnaire survey was conducted and the survey data, including main demographic information and smoking attempt, was analyzed by binary logistic regression test. Smoking attempt was defined as the teenagers had smoked at least one puff prior to the time of questionnaire survey. Results:A total of 3 630 valid questionnaires were collected and the rate was 96.9%(3 630/3 745). Overall 4.4%(158/3 630) of participants had smoking attempt. The results of binary logistic regression test showed that compared with women, men had higher risk of attempted smoking (P<0.05). Compared with registered residence in Shanghai, the risk of smoking behavior was higher among adolescents who were not registered residence in Shanghai (P<0.05). Smoking behavior among family members, classmates and friends were risk factors for adolescents to attempt smoking (P<0.05). When the number of family smoker was higher than 2, the risk of young students trying to smoke was significantly higher (P<0.05). Compared with the students who believed smoking was harmful to adolescents' health, it was a risk factor for those who considered trying to smoke might be harmful or harmless to try smoking (P<0.05). Conclusion:Smoking control in Jiading District schools has achieved some success, and the proportion of teenagers trying to smoke is low. However, further enhancement of smoking control among young people is still in need and we should devote more attention on male adolescents with non-Shanghai residence registration, and family smoke-free environment.

ABSTRACT

OBJECTIVE@#To understand the relationship between health literacy and patient experience of outpatients in China, and to explore its mechanism.@*METHODS@#The conceptual framework was developed based on Andersen's behavioral model of health services use and health literacy skills framework. An online cross-sectional survey was conducted with snowball sampling method, while the health literacy was measured by self-designed patient health literacy scale, and the patient experience was measured by the Chinese patient experience questionnaire for ambulatory care developed by Peking Union Medical College. And a structural equation model was built to explore the relationship between them and test the mechanism of health literacy influencing patient experience.@*RESULTS@#A total of 2 773 subjects were investigated. The average score of health literacy was (90.72±12.90) points, accounting for 78.89% of the full score, and the dimension of seeking social support had the lowest score. The average score of overall rating of patient experience was (3.71±0.74) points, and the scores of each dimension of patient experience were between 3.56 and 3.80. The model fit indices of structural equation model for overall rating of patient experience among the outpatients were χ2/df=9.29 (χ2=4 107.27, df=442), root mean square error of approximation (RMSEA)=0.055 (< 0.06), comparative fit index (CFI)=0.926 (>0.90), Tucker-Lewis index (TLI)=0.918 (>0.90), standardized root mean square residual (SRMR)=0.061 (< 0.08), the model was acceptable. The variance in patient experience explained by the model was 0.108. The structural equation model analysis results showed that the overall rating of outpatient experience was directly affected by health literacy (β=0.263, P < 0.001), also indirectly affected by health literacy (β=0.012, P < 0.001). In other words, the overall rating increased by 0.275 units for each standard deviation increase of health literacy. Self-evaluated health status mediated the relationship between health literacy and the overall rating of outpatient experience. In terms of diffe-rent dimensions of patient experience, the standardized path coefficient of the total effect of health literacy on patient experience was as follows: Information guidance 0.337, humanistic care 0.319, communication with doctors 0.294, service efficiency 0.240, and hospital environment 0.173.@*CONCLUSION@#The patients with higher level of health literacy were more likely to have a better outpatient experience in China, and the information guidance experience and humanistic care experience were most affected by health literacy. And the communication and information utilization ability had the greatest influence on patient experience.

ABSTRACT

ObjectiveTo investigate the role of hepatitis E virus nucleic acid (HEV RNA), hepatitis E virus antigen (HEV Ag), hepatitis E virus antibodies (HEV IgM antibody and HEV IgG antibody) in the clinical diagnosis of hepatitis E virus infection. MethodsA total of 13 992 specimens, for which the detection of HEV IgM antibody and HEV IgG antibody were performed in Peking University People’s Hospital, were collected, among which 1924 had positive HEV IgM antibody or HEV IgG antibody. Quantitative real-time PCR was used to determine HEV RNA and HEV genotype, and enzyme-linked immunosorbent assay was used to detect HEV Ag, HEV IgM antibody, and HEV IgG antibody and measure the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBil), and direct bilirubin (DBil). The non-parametric rank sum Mann-Whitney U test was used for comparison of continuous data between groups. ResultsAmong the 1924 specimens with positive HEV IgM antibody or HEV IgG antibody, 152(7.9%) had positive HEV IgM antibody, 1897(986%) had positive HEV IgG antibody, 62(3.2%) had positive HEV RNA, and 55(2.9%) had positive HEV Ag. Among the 152 specimens with positive HEV IgM antibody, 62(40.8%) had positive HEV RNA, and 55(36.2%) had positive HEV Ag, and among the 62 specimens with positive HEV RNA, 55(88.7%) had positive HEV Ag. The HEV RNA-positive group with 62 specimens had significantly higher levels of ALT, AST, TBil, and DBil than the HEV RNA-negative group with 90 specimens (Z=-7.609, -6.942, -5.815, and -6.130, all P<0.001), and the HEV Ag-positive group with 55 specimens had significantly higher levels of ALT, AST, TBil, and DBil than the HEV Ag-negative group with 97 specimens (Z=-6.413, -5.786, -5.199, and -5.545, all P<0.001). Nested PCR amplification and sequencing showed that in the HEV RNA-positive group with 62 specimens, 58 had positive HEV RNA and 4 had negative HEV RNA. The high level of positive HEV IgG antibody significantly reduced the S/CO value of HEV Ag detection and even yielded a negative result. ConclusionCompared with HEV antibodies, HEV Ag can improve the diagnostic level of hepatitis E and has certain clinical significance. High levels of aminotransferases or bilirubin can help with the diagnosis of hepatitis E. The high level of HEV IgG antibody may reduce the detection value of HEV Ag, and attention should be paid to HEV IgG antibody value when HEV Ag is negative.

ABSTRACT

Prostate cancer is one of the most common malignancies in men. Ablation therapy represented by cryoablation is effective in treating prostate cancer. However, currently, how to improve long-term curative effect and reduce local recurrence are the main problems. Ablation therapy can activate the body's anti-tumor immune response, in order to interfere with residual, tumor recurrence and distant metastatic lesions after ablation. The research advancements of ablation combined with immunotherapy for prostate cancer were reviewed in this article.

ABSTRACT

Objective: To evaluate the efficacy and safety of sedation using oral midazolam in combination with intranasal dexmedetomidine for magnetic resonance imaging (MRI) in paediatric patients. Methods: A total of 186 children aged 3-8 years undergoing MRI in Shanghai Children's Medical Center, Shanghai Jiao Tong University School of Medicine, from March 2019 to July 2019, were randomly assigned to two groups, who received either oral 50 mg/kg chloral hydrate combined with 2 μg/kg intranasal dexmedetomidine (chloral hydrate group, n=93) or oral 0.3 mg/kg midazolam combined with 2 μg/kg intranasal dexmedetomidine (midazolam group, n=93). The efficacy and safety of the sedative regimens were evaluated based on the successful sedation rate, sedation onset time, wake-up time, total sedation time and the incidence of clinical side effects. Results: The successful sedation rates were 98.9% and 94.6% in chloral hydrate group and midazolam group, respectively, with no significant difference (P>0.05). The sedation onset time was 20.0 (10.0, 20.0) min in midazolam group, which was longer than that in chloral hydrate group (P=0.000). The wake-up time was 15.0 (3.0, 31.8) min and the total sedation time was 68.0 (58.0, 82.0) min in midazolam group, both of which were significantly shorter than those in chloral hydrate group (P=0.000, P=0.001). The sedation in midazolam group was better accepted by children who did not experience nausea or vomiting. Hypotension and bradycardia were rare and did not require intervention in either group. Conclusion: Oral midazolam combined with intranasal dexmedetomidine is better accepted by the paediatric patients and the wake-up time of it is shorter, which may be used effectively and safely for sedation during paediatric MRI procedures.

ABSTRACT

This research explored the synergistic effects and the mechanism of parthenolide (PTL) and vorinostat (suberoylanilide hydroxamic acid, SAHA) on the proliferation of A549 non-small cell lung cancer cells. The combination effect of PTL and SAHA was detected by cell counting kit-8 (CCK-8) and colony formation assays. Scratch test was performed to detect cell migration. Annexin V-fluorescein isothiocyanate isomer/propidium iodide (FITC/PI) flow cytometry and Western blot analyses were used to determine cell apoptosis and its mechanism. The results showed that combination of PTL and SAHA inhibited the proliferation and migration of A549 with a synergistic effect compared to the single-drug groups. The combination of PTL and SAHA had synergistic effect to induce cell apoptosis by regulating p53 and c-myc pathways, and affected the expression levels of poly ADP-ribose polymerase (PARP), cysteinyl aspartate specific proteinase (caspase)-9, and caspase-3. Taken together, this study shows that combination of PTL and SAHA has synergistic effect, induces cell apoptosis and inhibits A549 proliferation, which is likely to be a novel strategy for the treatment of non-small cell lung cancer.

ABSTRACT

Objective The relationship between calcified nanoparticles (CNPs) and the formation of urinary stones is drawing increasing attention and the specific mechanisms involved. This study aims to investigate the mechanisms of the formation of kidney stone caused by CNPs. Methods A total of 48 rats were randomly and equally divided into a CNPs group (each rat was injected with 2 mL CNPs through the tail vein to establish a rat kidney stone model of CNPs), and a control group (injected the same amount of sterile isotonic saline instead of CNPs). We compared the expression levels of autophagy-related proteins, such as Beclin-1 and LC-3, the formation of autophagosomes and calcium salt crystals in renal tissues at time points of 3h, 6h, 12h, 24h, 1w, 2w, 4w and 8w in two groups. Results The relative expression levels and positive cells of Beclin-1 and LC-3 in CNPs group at 3h,6h,12h,24h, 1w, 2w, 4w, 8w were significantly higher than those in the control group (P< 0.05), and reached the highest value at 24 (P< 0.05). The number of autophagosomes at 24h, 1w, 2w, 4w, and 8w in the CNPs group ((2.83±0.32), (3.00±0.26), (3.70±0.44), (3.90±0.98), (4.70±0.51)/HP, respectively) were significantly higher than those in the control group (0.73±0.15)/HP (P <0.05). The scores of calcium salt crystals in the CNPs group at 2w, 4w, and 8w significantly increased compared to the control group (P <0.05). The calcium salt crystal formation score ((0.92 ± 0.98) points) was positively correlated with the expression intensities of Beclin-1 and LC-3 ((6.78 ± 4.25), (2.61 ± 2.57), respectively) (r = 0.843, 0.628, P <0.05), which was positively correlated with the number of autophagosomes (2.53 ± 1.41) (r = 0.923, P <0.001). Conclusion CNPs may damage renal tubular epithelial cells, and induce immediate autophagic activity, also increase expression of autophagy-related proteins and autophagosome formation, which will promote the formation and aggregation of calcium salt crystals in renal tubules to some extent.

ABSTRACT

Objective To explore the path and effect of family cohesion, family adaptability and health remind on health behavior improvement of chronic disease patients, so as to provide evidences for the family-centered chronic disease management. Methods Data of all 1 134 patients with chronic diseases was selected from the national sample survey database. t-test was used to compare family cohesion, family adaptability, health remind and health behavior improvement between different groups, and Pearson correlation was used to analyze the correlation between variables. In the process of path analysis, parameters were estimated by maximum likelihood. Results Among the 1 134 chronic diseases patients, male accounted for 41.4%, and female accounted for 58.6%. The average age of participants was (53.6±10.0) years old. The score of health behavior improvement was (1.89±1.20). Path analysis showed that family cohesion had indirect effects on behavior improvement (effect size=0.072); family adaptability had both direct and indirect effects on behavior improvement (effect size=0.156); Health remind had direct effect on behavior improvement (effect size=0.357). Conclusions The high-level family cohesion and adaptability can improve patients’ health behavior by improving the health remind. It is suggested that family-centered health management services for chronic disease patients should emphasize the participation of family members as well as the improvement of family function.

A total of 4 schools were randomly selected from primary schools which had more than 70% students with rural resident registration in each selected county, and 3 classes were randomly selected from fifth and sixth grades of each selected school. All students in each selected class were included in this study. A total of 2 521 students were included, and 2 506 valid questionnaires were collected with an effective response rate about 99.4%. Self-designed questionnaires were used to collect data on individual information, family information, and the incidence of school violence with related factors. Multivariable logistic regression was conducted to analyze related factors of school violence.@*Results@#The incidence of violence in rural primary schools was 50.4% (1 264/2 506) in 2016. The number of students who had physical assault, fought or been bullied was 682 (27.2%), 689 (27.5%) and 764 (30.5%), respectively. The results of multivariable logistic regression showed that the OR (95%CI) values of incidence of school violence for male students was 2.13(1.78-2.54) compared with female students; and the OR(95%CI) values of incidence of school violence for students with high peer acceptance, high family intimacy, more physical activity and mother who had education background of junior high school, senior high school, technical secondary school and junior college, bachelor′s degree or above were 0.39 (0.26-0.57), 0.47 (0.40-0.57), 0.64 (0.52-0.78), 0.79 (0.66-0.96), and 0.65 (0.46-0.92) compared with those with low peer acceptance, low family intimacy, less physical activity and mother who had primary education and below.@*Conclusion@#The incidence of violence in rural primary schools is not optimistic. Gender, peer acceptance, family intimacy, physical activity and mother education are related to school violence.

ABSTRACT

Objective To investigate the epidemic situation and its epidemic characteristics of malaria in Qingdao City from 2012 to 2017, so as to provide the evidence for the prevention and control of malaria in this city. Methods The data of cases and epidemiological characteristics of malaria in Qingdao City, 2012–2017 were collected from the China’s disease prevention and control system infectious disease report information management system and parasitic disease prevention and control information management system, and analyzed. Results A total of 103 cases of malaria were reported in Qingdao City from 2012 to 2017, and all of them were imported. Among them, 98 (95.15%) were imported from Africa and 5 (4.85%) from Asia. There were 92 laboratory confirmed cases (89.32%) and 11 (10.68%) clinically diagnosed cases. The highest incidence occurred in 2016 (0.28/105) . According to the classification of cases, there were 87 falciparum malaria cases (84.47%), 13 vivax malaria cases (12.62%), and 3 ovale malaria cases (2.91%) . These patients were mainly young men, and the male to female ratio was 19.6∶1. There were malaria case reports in each month, and the peak onset season was not obvious. Newly diagnosed and confirmed units were mainly municipal medical institutions; the median of onset-definite diagnosis was 4 days. There was one death case, but there were no recurrence, reburning, and imported secondary cases. Conclusion There were no local malaria cases in Qingdao City from 2012 to 2017. The prevention and control of imported malaria is the focus of the future work.

ABSTRACT

Calcific uremic arteriopathy (CUA), termed calciphylaxis, is a rare but highly fatal clinical syndrome. There is no clearly laboratory diagnostic criteria for CUA. The medium and small arterial calcification and microthrombosis discovered by skin biopsy, radiologic imaging, bone scan and the evidence of activation of the bone morphogenetic protein signal (BMPs) transduction pathway are useful for early diagnosis of this disease. The common therapies (including intravenous sodium thiosulfate (STS) and bisphosphonates, hyperbaric oxygen therapy and other symptomatic supports) are used for the management of wounds, pain, nutrition, dialysis and so on. Controlling the chronic kidney disease-mineral and bone disorder (CKD-MBD) and some complications of dialysis and drugs (such as warfarin, active vitamin D) can prevent CUA. However, CUA patients still have poor prognosis and high mortality. Since some patients progress rapidly, it is of great importance to make early diagnosis and provide effective treatments with multidisciplinary management.

ABSTRACT

A new sensitive method for detection of cumulative radon radiation based on the lead was established. PS2.M which stabilized by K+with hemin as a co-factor exhibits superior peroxidase-like activity, and can effectively catalyze the H2O2-mediated oxidation of TMB. In the presence of Pb2+, K+-stabilized PS2.M DNAzymes are induced to undergo a conformational change, because Pb2+has a higher efficiency with regard to stabilizing G-quadruplexes than K+, accompanied by a decrease of catalytic activity and a sharp decrease of readout signal. In the work, a novel"turn-off"model of colorimetry-Pb2+biosensor based on superior peroxidase-like activity of G-quadruplex for Pb2+analysis was developed. The fading degree of reaction system (△A value) was linearly related to Pb2+concentration in the range of 5. 0×10-9-1. 8× 10-7mol/L. The linear regression equation was △A=0.36+0.13C (10-8mol/L), with R=0.9987. The detection limits of lead and radon were 3.76 nmol/L(S/N=3) and 1.96×103Bq·h/m3(S/N=3), respectively. The method exhibited good selectivity, high sensitivity and convenient operation, and could avoid the radioactive hazard in determination of the radon in environment.
